Transition Services

Exit Assistance

As a Pathways resident, you are eligible for monetary exit assistance when transitioning out of the program.

Advanced Independence Training (AIT)

Successful program participants can participate in the AIT housing component of Pathways. AIT recipients will meet with their Advocacy Counselor once a month, participate in Life Skills and Pathways activities and also receive rental assistance equal to 50% of their monthly rent, for a maximum of six months.

Alumni Services

Rental Assistance

To receive rental assistance you must make the Youth Advocate aware of your housing situation and initiate a formal request for funds. You must provide proper documentation (i.e., apartment lease agreement and/or a letter from the person you will reside with) along with the monthly rental amount. Please note that this request takes about two weeks to process. Whenever possible, contact the Youth Advocate before your rent is due.

Bus Pass

Contact the Youth Advocate to request a monthly bus pass. Alumni can access a bus time three times after you leave the program. Make arrangements with the Youth Advocate to pick the bus pass up from UFC’s downtown office or one of the program sites.

Emergency Funds

Alumni can access emergency funds when an unexpected hardship occurs (i.e. car breaks down, loss of job, groceries, etc.) and will negatively affect your ability to survive independently. If you have an emergency situation, contact the Youth Advocate immediately to make a formal request for funds.

Career Development

The Career Development Department is available to assist you with resume writing, internships, interview preparation, job search support & maintenance, career exploration and individual career counseling. To access these services, the Youth Advocate can connect you to the appropriate staff, or you can contact the Career Development Manager directly.

Mental Health

Pathways Alumni can continue to access mental health services after they move out of the program. Contact the Youth Advocate to set up an appointment with one of the on-site therapists.

Educational Services

Educational services are available to current residents and program alumni. The Career Development Department can also assist alumni with college enrollment, vocational training opportunities, scholarship information, transcript review, course selection and individual educational counseling. To access these services, the Youth Advocate can connect you to the appropriate staff, or you can contact the Career & Education Counselor directly.

Furniture Expenses

The Alumni Services Department can make funds available to you should you need assistance purchasing furniture and/or other household items for your apartment. Contact the Youth Advocate to determine the amount monetary assistance you are eligible to receive.

Beyond the career services and college counseling I received, Pathways was always there when no one else was and has become the family I never had. When I’m home visiting from school, it’s Pathways I return to and see friends and staff members and participate in the alumni activities."
